Proof of COVID-19 Vaccine now available on-line

The Public Safety Commission and the Kateri Memorial Hospital Centre (KMHC) wish to inform the community that you can now obtain proof of your COVID-19 vaccination via the Quebec Government website.

Those who wish to access their vaccination certificate can visit https://covid19.quebec.ca/PreuveVaccinale for a digital copy that can be saved to your smartphone, tablet, computer, or printed for your records. Just fill out the required information and follow the step-by-step instructions.

Please note that this is a Quebec initiative, and the KMHC does not yet know at this time what the QR Code will be used for.

While it is recommended to download this proof of vaccination, please ensure to keep your original vaccine certificate that was issued by the KMHC at the time of your vaccine.
